Dear patient, we warmly welcome you as a (new) client of our practice!
We would like to draw your attention to our house rules.
• During our initial consultation, we will collect the necessary information to assess your current oral health and discuss your wishes. You will need to complete a health questionnaire to ensure we meet your expectations as effectively as possible.
• Please bring valid identification and a health insurance card to your first visit.
• Treatments are by appointment only.
• In case of emergencies or if you are experiencing pain, please call us before 10:00 AM, preferably, so we can try to help you the same day. Please adhere to the appointment time offered to you. We will then be unable to accommodate your work/school preferences.
• It is possible that a treatment may run over, for example, if the dentist has to treat an emergency or a treatment proves more intensive than anticipated. You may therefore have to wait a while for your appointment. The dentist or dental hygienist cannot be held responsible for any delays. You can help us prevent delays by arriving on time for your appointment.
• When visiting the practice, please silence your mobile phone.
• Please inform us of any changes to your insurance, (email) address, or telephone number in a timely manner. You can also do this via the contact form on this website.
• If there are any changes regarding your health or medication use, please inform your dentist or dental hygienist at your next visit. It is very important to report this to prevent complications during your treatment.
• You must visit the practice at least once a year for a dental checkup (this also applies to patients with complete dentures). For patients with natural dentition and for children, we recommend twice a year for checkups.
• We send an email reminder to our patients a few days before their appointment. This is a free, no-claims service. If you don't receive a reminder from us, this doesn't mean your appointment has been canceled.
• If you miss an appointment, or if the appointment is canceled less than 24 hours in advance, we may charge you for the missed appointment. The so-called code C90 may be used if an appointment is not canceled 48 hours in advance. You may be charged up to 100% of the missed revenue (reserved time). This means an average of €45 per 15 minutes of reserved time. These costs are not reimbursed by your health insurer; you must pay them yourself.
• If you need to cancel an appointment, please do so by phone or in person at the reception desk two business days in advance.
• We reserve the right to deregister patients who regularly miss their appointments.
• We use the billing company Infomedics for our billing. In some cases, we request immediate payment before treatment (for example, in the case of an emergency for a patient who is not a registered patient or for patients without dental insurance). The patient is responsible for ensuring payment is made as agreed. More information about the payment terms can be found on our website.
• We strive to ensure a clean mouth. Please brush your teeth before your appointment.
• Aggressive behavior, verbal or otherwise, will not be tolerated and will result in immediate removal from the practice.
• Please take care of your own belongings. Smile Dent Dental Practice is not liable for damage, theft, or loss of your belongings. You are advised not to leave your belongings unattended.
• As a patient, you have the right to protection of your privacy. At Smile Dent Dental Practice, all data necessary for a patient's treatment is recorded and stored in accordance with the Personal Data Protection Act. The practice protects your privacy in accordance with this law and handles your personal and medical data with care. More information can be found in our privacy statement.
• Dentists and other employees of Smile Dent Dental Practice are bound by professional secrecy or confidentiality. This means that they may not provide information about your treatment to individuals or organizations not involved in the treatment without your permission.
